SECTION 1 INTRODUCTION SCOPE AND WHO WE ARE

Connected Health Solutions, LLC, doing business as COHealth ("COHealth," "we," "us," or "our"), owns and operates CarebidetUSA.com (the "Website"). This Privacy Policy explains how we collect, use, disclose, and protect personal information when you:

• Visit or use the Website;

• Complete a questionnaire or online form;

• Contact us or request information;

• Schedule or participate in a consultation;

• View videos or other embedded content;

• Download materials; or

• Register for or use our password-protected training program.

COHealth is the exclusive United States master distributor and reseller of Carebidet. Carebidet is manufactured by Curaco Inc. ("Curaco"), a South Korean company that also holds a minority ownership interest in COHealth. COHealth and Curaco are separate legal entities. We may share limited information with Curaco when reasonably necessary for product support, technical assistance, quality assurance, safety, warranty, regulatory, or related operational purposes, as explained in this Privacy Policy.

The Website does not currently process online purchases or offer paid subscriptions. If we introduce online purchasing, payment processing, or additional paid services in the future, we will update this Privacy Policy and provide any additional notices required at that time.

This Privacy Policy applies only to information processed by or on behalf of COHealth. It does not apply to independently operated third-party websites, including curaco.kr, even when the Website contains a link to them. Those websites are governed by their own privacy policies.

SECTION 2 INFORMATION WE COLLECT

We collect personal information from Website visitors, individuals seeking information about Carebidet, family members and caregivers, healthcare and care-management professionals, representatives of healthcare organizations, training participants, and other individuals who communicate with COHealth.

Information You Provide

Depending on how you interact with us, you may provide:

• Contact information, such as your name, email address, telephone number, mailing address, and preferred method of contact;

• Professional or organizational information, such as your employer, organization, job title, professional role, or type of care setting;

• Inquiry and consultation information, including the person for whom you are seeking information, the current care setting, toileting or transfer assistance needs, whether the individual is primarily bed bound, current toileting methods, and the type of assistance requested;

• Health- or care-related information that you voluntarily provide, such as a general medical condition, mobility limitation, care need, or information relevant to considering whether Carebidet may be appropriate;

• Communications, including information contained in emails, telephone conversations, consultation requests, questionnaires, support requests, and other correspondence;

• Training information, such as your name, email address, username, organization, course enrollment, training progress, quiz responses, scores, completion status, and certificate information; and

• Other information you voluntarily provide to us.

The Website questionnaire requires only your first name and email address. You may voluntarily provide your full name, telephone number, organization, care setting, general condition or care need, mobility or toileting information, and other details that may help COHealth respond. Optional questions may be skipped.

Please provide only the information reasonably necessary for your inquiry. Do not submit Social Security numbers, payment-card information, financial-account information, government identification numbers, complete medical records, or other highly sensitive information through ordinary Website forms or email.

Information Collected Automatically

When you visit or use the Website or training platform, we and our service providers may automatically collect limited technical and usage information, including:

• Dates and times of access;

• Pages viewed, links selected, and files downloaded;

• Referral information and the pages used to reach the Website; and

• Cookie, analytics, session, and similar technical identifiers.

Sources of Information

We may collect information directly from you; from a family member, caregiver, legal representative, healthcare professional, care manager, or organization acting with your authorization; automatically through the Website and related technologies; and from service providers that help us operate our forms, scheduling, communications, analytics, customer records, or training systems.

SECTION 3 HOW WE USE PERSONAL INFORMATION

We may use personal information to:

• Respond to inquiries, questionnaires, consultation requests, and requests for information;

• Provide general information about Carebidet and help individuals and care teams consider whether it may be appropriate for a particular situation;

• Schedule and conduct consultations, demonstrations, evaluations, training, or other requested activities;

• Create and manage password-protected training accounts;

• Deliver training materials, record course progress and quiz results, and issue or verify completion certificates;

Health- or care-related information submitted through the Website is used to understand the inquiry, provide general Carebidet information, and coordinate requested assistance. COHealth does not use Website submissions to diagnose or treat medical conditions.

If you receive promotional email communications from us, you may unsubscribe by using the link provided in the message or by contacting us at info@cohealth.solutions. We may still send non-promotional communications relating to an inquiry, consultation, training account, product support request, safety matter, or other existing relationship.

We will obtain consent before sending marketing text messages when consent is required by applicable law. Consent to receive marketing communications is not a condition of obtaining information about Carebidet.

SECTION 4 WHEN AND WITH WHOM WE DISCLOSE PERSONAL INFORMATION

COHealth does not sell or rent personal information. We also do not disclose personal information to third parties for cross-context behavioral advertising.

Service Providers

We use service providers to operate the Website, collect and route inquiries, manage communications, schedule consultations, provide videos, maintain business records, and operate the training program. Depending on the features you use and the technology currently deployed, these providers may include:

• The Website's hosting provider (currently Railway), for Website hosting and related Website functions;

• Tally, for questionnaires, contact forms, and information submissions;

• Make, for securely routing and automating submitted information between systems;

• HubSpot, for customer-relationship management, inquiry records, and communications;

• Calendly, for consultation and meeting scheduling;

• Vimeo, for hosting and displaying videos;

• Microsoft, for email, documents, communications, and business productivity services;

• Google, where used for Website analytics or related technical services;

• IONOS, where used for hosting, infrastructure, domain-related services, or the training environment; and

• Moodle or MoodleCloud, where used to provide training accounts, course content, quizzes, progress tracking, and completion certificates.

These providers may process personal information only as necessary to perform services for COHealth or as otherwise described in their applicable privacy notices. The particular providers involved depend on how you interact with the Website and how the applicable service is configured.

Curaco

Curaco is the manufacturer of Carebidet and holds a minority ownership interest in COHealth. Curaco and COHealth are separate legal entities. We may disclose limited information to Curaco when reasonably necessary for product support, technical assistance, quality assurance, safety review, warranty coordination, regulatory activities, or related operations. Curaco does not receive unrestricted access to all Website inquiries or COHealth records merely because of its ownership interest.

At your request or with your permission, we may communicate with a healthcare provider, care manager, facility, home-care organization, insurer, financing resource, government program, or other person involved in evaluating or coordinating Carebidet for you or another individual.

Professional Advisers and Operational Support

We may disclose information to attorneys, accountants, insurers, consultants, contractors, and other professional advisers when reasonably necessary to operate our business, obtain advice, protect our rights, or comply with our obligations.

We may disclose information when we reasonably believe disclosure is necessary to comply with applicable law, a subpoena, court order, or other legal process; respond to a government request; investigate suspected fraud or misuse; protect the security of our systems; or protect the rights, property, health, or safety of COHealth, Website users, or others.

Business Transactions

Information may be disclosed or transferred as part of an actual or proposed merger, financing, investment, acquisition, reorganization, sale of assets, or similar business transaction, subject to appropriate confidentiality and legal protections.

We may disclose information to another person or organization when you direct us to do so, request the disclosure, or provide consent.

SECTION 5 COOKIES ANALYTICS AND EMBEDDED CONTENT

The Website and training platform may use cookies, pixels, local storage, log files, and similar technologies. These technologies allow COHealth and its service providers to:

• Operate and secure the Website and training platform;

• Maintain sessions and training-account access;

• Remember certain settings or preferences;

• Understand Website traffic and use;

• Identify technical problems and improve Website performance;

• Process forms and scheduling requests; and

• Display videos and other third-party content.

Some cookies are necessary for the Website or training platform to function. Other cookies may support analytics, performance measurement, or third-party features.

Analytics

COHealth may use Website analytics provided through the Website's hosting provider, Google Analytics, or similar services to understand how visitors find and use the Website. Analytics information may include Internet Protocol address, browser and device information, approximate location derived from an Internet Protocol address, pages visited, referring pages, and dates and times of access. We do not use Website analytics to obtain your precise physical location.

Third-Party Features

When you view an embedded Vimeo video, use a Tally form, schedule through Calendly, or interact with another embedded third-party feature, that provider may receive technical and usage information and may use cookies or similar technologies. Its handling of that information is also governed by its own privacy notice.

Most browsers allow you to delete or block cookies or notify you before a cookie is stored. Blocking cookies may affect certain Website or training-platform functions. Where a cookie preference tool is provided, you may use it to manage the available choices.

Do Not Track and Global Privacy Control

Some browsers offer Do Not Track or Global Privacy Control signals. Because COHealth does not sell personal information or use it for cross-context behavioral advertising, the Website does not currently provide a separate sale-or-sharing opt-out mechanism. The Website may not respond separately to Do Not Track signals. We will honor legally recognized browser-based privacy signals if and when applicable law requires us to do so.

SECTION 6 DATA RETENTION AND SECURITY

Data Retention

COHealth retains personal information for as long as reasonably necessary to fulfill the purposes described in this Privacy Policy, including to:

• Respond to and maintain records of inquiries and consultations;

• Provide training and manage training accounts;

• Record course progress and verify completion certificates;

• Provide product support and coordinate warranty, quality, or safety matters;

• Maintain appropriate business and communications records;

• Protect Website and system security;

• Resolve disputes and enforce agreements; and

• Comply with legal, regulatory, tax, insurance, and recordkeeping obligations.

The applicable retention period depends on the nature of the information, why it was collected, whether an ongoing relationship exists, and any applicable legal or operational requirements. Information may remain temporarily in system backups after it is removed from active records.

When personal information is no longer reasonably required, COHealth may delete, anonymize, or securely dispose of it, subject to applicable legal and operational requirements.

Information Security

COHealth uses reasonable administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, loss, misuse, alteration, or disclosure. We also rely on security measures maintained by the service providers that operate portions of the Website, forms, communications, customer-relationship management system, and training platform.

No website, email transmission, online form, or electronic storage system can be guaranteed to be completely secure. Accordingly, we cannot guarantee the absolute security of personal information.

Website forms and ordinary email are not intended for medical emergencies, urgent clinical communications, or the transmission of complete medical records, Social Security numbers, payment-card information, financial-account information, government identification numbers, or similarly sensitive information.

If you believe that information you provided to COHealth may have been accessed or used without authorization, please contact us promptly at info@cohealth.solutions or +1-407-550-0301.

SECTION 7 YOUR PRIVACY RIGHTS AND CALIFORNIA NOTICE

Your Privacy Rights and Choices

You may contact COHealth to:

• Ask whether we maintain personal information about you;

• Request access to personal information you have provided;

• Request correction of inaccurate or incomplete information;

• Request deletion of personal information;

• Withdraw a consent you previously provided, where applicable; or

• Opt out of promotional email or text communications.

To submit a request, contact info@cohealth.solutions or call +1-407-550-0301. Please describe your request with enough detail for us to identify the relevant information.

We may take reasonable steps to verify your identity and authority before acting on a request. We may be unable to fulfill a request completely when retaining or using the information is reasonably necessary to provide a requested service, maintain training or certificate records, address product safety or warranty matters, comply with law, protect security, resolve disputes, exercise legal rights, or protect the rights of another person.

Where applicable law provides additional privacy rights, COHealth will process qualifying requests in accordance with that law. An authorized agent submitting a request may be required to provide evidence of authorization, and we may contact the individual directly to verify the request.

California Residents

This Privacy Policy describes the categories of personal information COHealth collects, the purposes for which it is used, and the categories of third parties to which it may be disclosed.

COHealth does not sell personal information and does not disclose personal information to third parties for cross-context behavioral advertising.

Based on its current operations, COHealth does not meet the statutory thresholds that generally determine whether a business is subject to the California Consumer Privacy Act. Accordingly, this Privacy Policy does not represent that COHealth is presently a covered Business under that law. If COHealth's operations or applicable legal requirements change, we will revise this Privacy Policy and provide any additional notices or rights required by law.

California residents may nevertheless submit the access, correction, or deletion requests described above. COHealth will consider and respond to those requests in accordance with its practices and any applicable legal requirements.

Changes to Your Information

If you have a training account, certain information may be updated through the account or by contacting COHealth. You may also contact us to request correction of information submitted through an inquiry, questionnaire, consultation request, or other communication.

SECTION 8 CHILDREN'S PRIVACY

The Website is intended for a general adult audience and is not directed to children under 13. COHealth does not knowingly collect personal information directly from children under 13.

You must be at least 18 years old to create or use a password-protected training account unless COHealth has expressly authorized another arrangement with a parent, legal guardian, educational institution, or other responsible organization.

Individuals under 18 should not submit personal information through the Website without the involvement and permission of a parent or legal guardian.

An adult may submit limited information concerning a child or other dependent when seeking general Carebidet information, provided that the adult is authorized to provide the information. Only information reasonably necessary for the inquiry should be submitted.

If we learn that personal information was submitted directly by a child under 13 without appropriate parental consent, we will take reasonable steps to delete it as required by applicable law. A parent or legal guardian who believes a child has provided personal information may contact us at info@cohealth.solutions or +1-407-550-0301.

SECTION 9 THIRD-PARTY WEBSITES PARTNER SITES AND SOCIAL MEDIA

The Website may contain links to websites and services operated independently by third parties, including:

• Curaco's website at curaco.kr;

• Partner and care-resource websites;

• Financing, insurance, government-program, or grant resources;

• Research publications and educational resources;

• Social-media platforms; and

• Other external websites or services.

When you select a link and leave CarebidetUSA.com, the third party's privacy policy and terms govern its collection and use of information. COHealth does not control the privacy or security practices of independently operated third-party websites.

A link to a third-party website does not, by itself, mean that COHealth provides that third party with your personal information. If COHealth proposes to send information to a partner, healthcare organization, financing resource, or other third party on your behalf, we will do so at your request, with your permission, or as otherwise disclosed in this Privacy Policy.

If you interact with COHealth through a social-media platform, COHealth may receive the information you submit, such as your name, username, message, or other account information made available through that platform. The social-media company may separately collect and use information according to its own privacy policy.

Embedded services, such as Vimeo videos, Tally forms, and Calendly scheduling, are addressed in Section 5 above.

COHealth may collect health- and care-related information that you voluntarily provide, including general information about medical conditions, mobility, toileting or transfer assistance, bed-bound status, current care methods, and care settings.

Health- and care-related questions in the CarebidetUSA questionnaire are optional. COHealth does not request detailed medical records through the questionnaire, and the form instructs users not to provide them. Please provide only the information reasonably necessary for COHealth to respond to your inquiry.

We collect this information directly from you through questionnaires, consultations, emails, telephone conversations, support requests, and other communications. We use it to respond to your request, provide general Carebidet information, consider potential product suitability, arrange requested assistance, and support product quality, safety, warranty, and regulatory activities.

COHealth does not sell consumer health data or use it for targeted advertising.

Consumer health data may be processed by service providers that support our forms, communications, scheduling, hosting, and information systems. These providers may include Tally, Make, Microsoft, the Website's hosting provider, IONOS, and Moodle or MoodleCloud, depending on the service used. HubSpot receives limited contact, organizational, audience, and general inquiry information; COHealth does not intentionally place optional health answers in HubSpot unless appropriate sensitive-data functionality and safeguards are enabled. Limited information may also be disclosed to Curaco when reasonably necessary for requested technical support, quality assurance, product safety, warranty, or regulatory purposes.

Where required by applicable law, you may request confirmation of whether COHealth collects your consumer health data; request access to or deletion of that data; withdraw consent to future collection or disclosure; or request information about recipients. Additional information and instructions are provided in our Consumer Health Data Privacy Notice.

If you provide information about another person, you confirm that you are authorized to do so. You should avoid providing that person's full name or other identifying information unless it is reasonably necessary for the request.

HIPAA

COHealth is a Carebidet distributor and reseller, not a healthcare provider. The public Website, ordinary email, and general inquiry forms are not patient portals. Health-related information submitted directly through these channels may not be protected health information under HIPAA. If COHealth receives protected health information while acting in a capacity governed by HIPAA, that information will be handled according to the applicable legal and contractual requirements.

SECTION 11 UNITED STATES OPERATIONS AND INTERNATIONAL DATA TRANSFERS

COHealth is based in the United States, and CarebidetUSA.com is primarily intended to provide information and services concerning Carebidet within the United States.

Personal information collected through the Website is primarily controlled by COHealth in the United States. Some service providers may process or store information in the United States or other countries where they or their subprocessors operate.

Curaco is located in South Korea. When reasonably necessary for requested technical assistance, product support, quality assurance, safety, warranty, regulatory, or related operational purposes, limited information may be transferred to Curaco in South Korea. COHealth will limit such disclosures to information reasonably necessary for the applicable purpose and will obtain consent when required by applicable law.

Privacy and data-protection laws in other countries may differ from those in your location. COHealth uses reasonable measures intended to protect personal information as described in this Privacy Policy.

The Website is not presently directed toward offering goods or services to individuals located in the European Economic Area, United Kingdom, or other non-U.S. jurisdictions. COHealth does not intentionally monitor the online behavior of individuals in those jurisdictions. If our activities expand internationally, we will reassess and update this Privacy Policy and our privacy practices as necessary.

SECTION 12 CHANGES TO THIS PRIVACY POLICY AND CONTACT INFORMATION

Changes to This Privacy Policy

COHealth may update this Privacy Policy periodically to reflect changes in the Website, training program, service providers, business practices, or applicable legal requirements.

When we revise this Privacy Policy, we will update the Last Updated date at the top. If a change materially affects how we collect, use, or disclose personal information, we may provide additional notice through the Website, training platform, email, or another appropriate method.

Changes will apply prospectively from the stated effective date unless applicable law requires otherwise. We encourage you to review this Privacy Policy periodically.
